Jane* suffered from severe anxiety and depression for years. At 18 years old, she rarely left her house even prior to the COVID-19 pandemic. As a result, she was not going to necessary wellness appointments. Jane’s parents did not know what they could do to help their daughter. Then one of her doctors, recognizing Jane’s problem, referred her to C&FS’ Health Homes program.

Health Homes is a care management service that encourages service providers to communicate with one another so all needs for those under 21 years old are addressed in a comprehensive manner. The goal of Health Homes is to get the child the services they need to stay healthy.

Jane worked on learning to help manage and cope with her anxiety and depression She began seeing a counselor in her home so she could discuss her fears and set goals for the future. The goals started small, leaving the house once a week, but then grew to leaving the house on a near-daily basis. Jane’s parents joined some of the therapy sessions so the family could work on their communication skills.

Since joining Health Homes over two years ago, Jane has become more self-assured. She has been able to manage her anxiety and continues to keep up-to-date on her wellness visits. With her new set of skills, Jane plans to start cosmetology school and is actively looking for a part-time job. While she knows there is more work to be done, she is proud of what she has achieved so far and looks forward to what is out there for her.
