Life can challenge families. Our preventive services show families how to work together as a cohesive unit.
The Preventive Services Program aims at strengthening families, enabling them to empower themselves to better their lives for their families and the communities in which they live. Through a comprehensive treatment plan, the program works with families to prevent the placement of children in the foster care system and maintain a healthy family structure within the home. Families learn how to solve problems and cope with one another by attending individual and group counseling sessions in their home environment.
Preventive Services is a fully grant-funded program. As a result, clients do not pay a fee to participate in the program. Referrals to the program are made through the Erie County Department of Social Services at (716) 858-8000.
Wraparound
The Wraparound Program serves children and youth who are under the age of 18 and diagnosed with an emotional, behavioral, or mental disorder. The goal of Wraparound is to provide families with a community-based alternative to further system penetration for their children who may be at risk of out-of-home placement.
Through an individualized care model, Wraparound works to engage the child, family, and other care providers to address the unique needs and strengths of each family. This process includes the development of a crisis plan and a plan of care, customized to fit the needs of each family, and implementation and evaluation of the plan. It includes significant collaboration with all parties involved in the family’s treatment such as vendor services. Some of the vendor services offered through Wraparound include:
- Care coordination
- Family support
- Crisis response services
- Skill-building
- Intensive in-home services
- Tutoring
- Pre-vocational services
- Therapeutic recreation
- Assisting the family in exploring respite within their natural supports and immediate community
- Referrals to challenged-based camp and outward-bound experiences
