When the Jones* family was referred to the Child and Family Services Wraparound Program, their mother was preparing for a moment she had been working toward for months: bringing her four children home after their time in foster care. 

While it was a joyful step forward, it came with many challenges. Three of the children have complex developmental needs and the family was already struggling to manage medical appointments, school support and transportation on their own. 

That’s when Wraparound stepped in. The family was connected with a Family Peer Advocate who provided stability and guided the mother through each obstacle. Together, they completed SSI applications, coordinated essential medical appointments and worked with the school district to ensure each child received the support they needed. Over time, tasks that once felt unmanageable began to fall into place. 

At home, Wraparound helped the family build a more organized, supportive environment, creating smoother daily routines. To strengthen this foundation, the mother was also connected with a Health Homes Care Manager for ongoing, long-term support. 

“Seeing the children settle into a stable routine and receive the supports they need has been one of the most meaningful parts of this work,” shared Haley Radominski, High Fidelity Wraparound Coordinator. “Their progress reflects the strong foundation now in place for their long-term stability.” 

Today, the children remain connected to the specialists they need and are in school placements that provide appropriate services for each of them. After successfully transitioning out of Wraparound, the mother was granted sole custody of her children, reflecting her perseverance and deep commitment to their family. 

Through it all, the mother’s dedication never changed. She stayed engaged, built a strong support network and created a stable environment where her children could grow.
