Matching Gift Procedure

  1. Obtain a matching gift form (or web address) from your organization’s HR department.
  2. Mail the completed matching form to C+FS with your donation. If matching registration is online, email the matching confirmation number to matching@cfsbny.org.
  3. Your company will send the matching gift after C+FS verifies your request.

The C+FS Tax ID number is 16-1004825.
